Payment Policies

Appointment Guidelines

All visits are by appointment only. If visiting for the first time, please arrive 15 minutes early to complete required forms, or download them in advance. As a courtesy, we provide reminder calls for all upcoming appointments.

Cancellation Policy

Please provide at least 24 hours' notice to cancel or reschedule. Cancellations with less than 24 hours' notice or missed appointments without prior notification are subject to a $50.00 fee.
